A Night of Accolades!Last night we had the final Fire Commissioners meeting of the year and it was special to say the least. First, we honored the dedicated service of Commissioner Sam Patrick for his several years of service to the District. Please join us in thanking him for his time as a Fire Commissioner and wish him well in his endeavors to come.Secondly, following recent Lieutenants testing we had the opportunity to hold a badge pinning ceremony. Public Records Requests

Lewis County Fire District 5 will only release approved records in accordance with RCW 42.56.070. You will need to complete a Request for Public Records Form, and we will respond to you whether we have the record you requested. At that time, you will be advised of the cost for the record, which varies, depending on whether you wish to have a copy via Email, fax, printed, or electronic media such as a CD or DVD. When payment is received, the record will be released if allowable.

Protected health information will only be released to the patient or their designated appointee with a notarized signature.

How to make Public Records requests

1. You can request records in person at 105 2nd Ave SE, Napavine, WA

2. You can mail your request to Lewis County Fire District 5, PO Box 259, Napavine, WA 98565

3. You Can request a record via fax at (360) 262-3893
